Antimicrobial susceptibility testing (AST) Breakpoints (ISO 20776-1:2006) Values of parameters, such as MICs, on the basis of which bacteria can be assigned to the clinical categories

The WT-UL values were determined following the EUCAST principles for visual and statistical ECOFF setting (Table 1). The ECOFF finder programme was unable to set statistical WT-UL endpoints for 4/20 laboratory- and species-specific MIC distribu-tions because of a high number of MIC values clustering at the upper or lower end of the MIC range.

Antimicrobial susceptibility testing (AST) – EUCAST, CLSI, ECOFFs and Clinical Breakpoint determination Posted on August 21, 2015 by mdjkf EUCAST is increasingly becoming the dominant international standard for various reasons and its procedures and documentation, including breakpoint tables are available online for free. The European Committee for Antimicrobial Susceptibility Testing (EUCAST) recently proposed epidemiological cut-offs (ECOFFs) for a limited number of antimicrobial compounds and for Campylobacter jejuni and Campylobacter coli only. The variability in the results obtained with these methods hampers setting epidemiological cut-off (ECOFF) values and clinical breakpoints according to EUCAST guidelines.

Before using the EUCAST breakpoint tables - make sure you read this. There are changes in the breakpoint tables every year. Some may be difficult to understand or accept without having followed the development of and consultations on the changing definitions of the susceptibility categories, especially the change from the old "intermediate" (I) to the new "Susceptible, increased exposure" (I). ECOFF values are not the same as clinical breakpoint concentrations.

Epidemiologic Cutoff Values Epidemiologic cutoff values, abbreviated ECV (CLSI) or ECOFF (Eucast) , are measures of a drug MIC distribution that separate bacterial populations into those representative of a wild type population, and those with acquired or mutational resistance to the drug.

Using also the EUCAST ECOFF, a significant increase in the proportion of isolates above this ECOFF was observed: from 3.7% in 2017 to 7.6% in 2018 (p<0.0002).
